Community Service Hours

After completing the training, volunteers will begin work toward their certification. The final stage of this process is completion of the community service commitment. The volunteers responsibility will include a variety of activities designed to teach others health promotion and related skills. Volunteers are required to provide a minimum of 20 hours of program payback, within a 12-month period following completion of the initial program training. A Monthly Activity Report form must be completed and submitted to your County Extension Office each month. The reporting form is available in two formats. The excel file can be downloaded to your computer, completed and emailed to your County Extension Office or is available as PDF for printing, completing, and mailing or faxing to the county office.
