Instructions for completing the Provider In-Take form.

  1. Make sure you have a version of Adobe Acrobat Reader that will allow you to type in the form. You can get the current version of Acrobat Reader here: http://get.adobe.com/reader/
  2. Open the form.
  3. Complete the form and save it. You can click the "Save as PDF" button, or go to File > save as if you are in Internet Explorer.
  4. Email the form as an attachment to:
